Form 8962, Premium Tax Credit

You use this form to “reconcile” your premium tax credit for 2014. If you had Marketplace insurance and used premium tax credits to lower your monthly insurance payment in 2014, you’ll have to file this form with your 2014 federal income tax return.

This form includes details about the coverage you and household members had through the Marketplace in 2014. You’ll need it to complete Form 8962, Premium Tax Credit.

Health coverage exemptions
